How To Get Started

2	Complete Your Assessment An assessor visits you at home to determine your eligibility and care needs.
3	Contact Diligent Care Services Get in touch with us and we will explain the partnership and complete a referral to Trilogy Care.
4	Complete Onboarding Trilogy Care will onboard you and help you choose between Self-Managed or Fully Coordinated.
5	Start Receiving Services Your care plan and budget are shared with us, and we begin delivering your services as scheduled.
