Specialised Support Coordination

There are three levels of support coordination that Diligent Care Services can help to be included in your NDIS plan:

Support Connection – This is to build your ability to connect with informal, community and funded supports that enable you to get the most out of your plan and pursue your goals.

Support Coordination – For the coordination of supports: This is to assist you build the skills you need to understand and use your NDIS plan. Diligent Care Services support coordinator will work with you to ensure a mix of supports are provided to increase your capacity to maintain your relationships, manage your service delivery tasks, live more independently and be included in your community.

Specialist support coordination – This is a higher level of support coordination. It is for people with more complex situations and who need specialist support. Our specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of needed services.

At Diligent Care Services we work together with you and your family to match you with the most appropriate specialist support coordinator to ensure your unique NDIS goals are being achieved.

A support coordinator and a specialist support coordinator may sound similar and function in a way that look similar, but they look after different NDIS support categories. Where support coordinators will help you in planning and implementing your capacity building programs, in-home supports and community inclusion programs, specialist support coordination will come in if you have additional or more complex needs fundable by the NDIS. Some of these needs could include:

  • Psychological and mental health support
  • Ongoing support from a social worker
  • Accessing health, education or justice services

Diligent Care Services team can facilitate both regular NDIS support coordination and NDIS specialist support coordination, depending on what is most appropriate for you and your situation.
